Services:

Wellness visits: The Nurse will perform visits to check the wellbeing of clients who may have need assistance with managing their illness or disease, who may have a new diagnosis and need assistance with disease management, live alone or may be in a Nursing home or Rehab facility, may be resistant to more extensive care, have health conditions requiring close monitoring, or for clients seeking a proactive approach to their health and need monitoring. The Nurse will create a Care Guide to assist with managing the client's condition. The Nurse will also report updates, changes or concerns to the client’s representative and if in the home, also report changes to the Physician. For clients who live at home, the provider will assist with monitoring, logging, and reminding the client to take vital signs, weight (as needed. The client must have their own scale), blood glucose (as needed), and medication management (medication reminders, organization, and scheduling). For clients in a facility the Provider will sit with them, listen to them, and advocate for them.

Post Operative Visits: For Post operative clients who need additional monitoring after surgery. The Nurse will meet the client at their home, assist the client with getting settled in, monitor the client for infection and complications, monitor wounds, dressings, and drains, assist with monitoring, logging, and reminding the client to take vital signs, weight (as needed. The client must have their own scale), and blood glucose (as needed), medication management (medication reminders, organization, and scheduling), report changes to the client’s representative and/or physician, and assist with the reinforcement of discharge instructions provided by the physician to reduce pain, reduce hospital re-admission, and aid in healing. The Nurse will create a Care Guide to assist with managing the client's condition.

Post Hospitalization Visits: The Nurse will visit the client in the home after hospital discharge to assist with disease management (Including new or existing chronic disease management such as CHF, COPD, Diabetes, and Hypertension) by assisting with monitoring, logging, and reminding to take vital signs, weight (as needed. The client must provide their own scale), and blood glucose (as needed), medication management (medication reminders, organization, and scheduling), report changes to the client’s representative and/or physician, and reinforcement of discharge instructions provided by the physician to reduce complications, and hospital readmission. The Nurse will create a Care Guide to assist with managing the client's condition.

Dementia Management Visits: The Nurse will make a visit and provide teaching on Dementia, and how to manage the care of those with Dementia. The Nurse will create a Care Guide to assist with managing the client's condition.

On Demand and After hours On Demand Visits: For existing clients who have unscheduled visit requests. Based on Provider availability.

For 2 hours of assistance with Companion care: Added to any service above and includes engaging and interacting with the Client in the home or a facility, meal preparation or picking up meals, light housekeeping, laundry, organizing, prescription pick up, medication reminders, grocery pick up and putting groceries away (groceries must be ordered beforehand), setting up toiletries for AM/PM care, and fall prevention (The companion will walk side by side with the client, and always be near by the client to prevent falls.).

For 4 hours of assistance with Companion care (as listed above) or to have staff accompany the client to and from any appointments (including any type of appointment. For doctor’s appointments, the Provider will stay with the Client and assist with filling out forms, understanding physician’s orders and instructions, prescription pick up, and fall prevention). Added to any service above.
Visits: Total payment is due upon booking for one time visits. For weekly visits (1 or more visits per week), total payment for the 1st week is due upon booking, and then at the end of each week. For monthly visits (1 visit per month) payment is due upon booking for the 1st visit, and then at the beginning of each month before services begin.
Cancellation: The Client, Client’s Representative, or the Client’s Responsible Party and the Provider must cancel or reschedule any visit 24 hours or more in advance. If a visit is canceled 24 hours or more in advance by the Client, Client’s Representative, or the Client’s Responsible Party, a refund will be given to the payer for that canceled visit. If a visit is not canceled 24 hours or more in advance, a refund will not be issued for that canceled visit. If the Provider cancels a visit without rescheduling, a refund will be issued to the payer for that canceled visit. A visit will be rescheduled for any Inclement weather.
The Client, Client’s Representative, or the Client’s Responsible Party is responsible for the client's care or finding a replacement if needed, when the Provider is not there.
If the Provider is sent home early a refund will not be issued.
The Client, Client’s Representative, or Client’s Responsible Party and the Provider may terminate services for any reason at any time.
The Provider will not tolerate any forms of criminal activity, abuse, violence, sexual harassment, unsafe living conditions including mold, bug, or rodent infestation, racism, or discrimination. These issues will automatically result in immediate verbal termination of services. For unsafe living conditions including mold, bug or rodent infestation, proof must be shown from the appropriate company that these issues were resolved before the provider returns.
Acupressure Massage Services
A great way for guests to relax, and enjoy their time at the office, a party, your event, and more! Additional fees may apply depending on environment (Example: Outside, places with no heat/AC depending on outside climate, swimming pool/water park, and more!). No inappropriate language, gestures, or touching will be tolerated, and the sessions will be canceled.
2 hours: A practitioner will set up and give 10-20 minute massage sessions (Depending on how many guests you have) to your guests.

Cancellation: Please cancel or reschedule appointments 24 hours or more in advance. For cancellations 24 hours or more in advance, a refund will be given. If an appointment isn't canceled 24 hours or more in advance, no refunds will be given. If the Provider cancels an appointment, a refund will be given. The Provider will cancel appointments with no refund if the client is more than 15 minutes late. For Inclement weather, all appointments will be rescheduled.
