Concierge Nursing and Healthcare, LLC provides personalized care to clients in their homes or any place convenient to the client. We also have add on companion, pet, and massage services for clients during services.

Services:

  1. Wellness visits: The Nurse will perform visits to check the wellbeing of clients who need assistance with managing their illness or disease, who may have a new diagnosis and need assistance with disease management, live alone or may be in a Nursing home or Rehab facility, may be resistant to more extensive care, have health conditions requiring close monitoring, or for clients seeking a proactive approach to their health and need monitoring.  The Nurse will work with the client on the physician's instructions to create a Care Guide to assist with managing the client's condition. The Nurse will also report updates, changes or concerns to the client’s representative and if in the home, also report changes to the Physician. For clients who live at home, the provider will assist with patient teaching, monitoring, logging, and reminding the client to take vital signs, weight (as needed.  The client must have their own scale), blood glucose (as needed), assistance with PT/OT/ST exercises, and device and medication management (including assisting with emptying catheters/drains, application of compression stockings, medication reminders, organization, and scheduling).  For clients in a facility the Provider will sit with them, listen to them, and advocate for them.

  2. Post Operative Visits: For Post operative clients who need additional monitoring after surgery.  The Nurse will meet the client at their home, assist the client with getting settled in, monitor the client for infection and complications, monitor wounds, dressings, and drains, assist with monitoring, logging, and reminding the client to take vital signs, weight (as needed.  The client must have their own scale), blood glucose (as needed), assistance with PT/OT/ST exercises, and device and medication management (including assisting with emptying catheters/drains, application of compression stockings or equipment, medication reminders, organization, and scheduling), report changes to the client’s representative and/or physician, and assist with the reinforcement of discharge instructions provided by the physician to reduce pain, reduce hospital re-admission, and aid in healing. The Nurse will create a Care Guide to assist with managing the client's condition.  

  3. Post Hospitalization Visits: The Nurse will visit the client in the home after hospital discharge to assist with disease management (Including new or existing chronic disease management such as CHF, COPD, Diabetes, and Hypertension) by assisting with monitoring, logging, and reminding to take vital signs, weight (as needed.  The client must provide their own scale), blood glucose (as needed), and device and medication management (including assisting with emptying catheters/drains, application of compression stockings, medication reminders, organization, and scheduling), assistance with PT/OT/ST exercises, report changes to the client’s representative and/or physician, and reinforcement of discharge instructions provided by the physician to reduce complications, and hospital readmission. The Nurse will create a Care Guide to assist with managing the client's condition. 

  4. Dementia Management Visits: The Nurse will make a visit and take vital signs, provide teaching on Dementia, and how to manage the care of those with Dementia. The Nurse will create a Care Guide to assist with managing the client's condition.

  5. Medication Management Only: This includes taking vital signs, Medication Reconciliation, patient teaching, calling prescription refills, picking up prescriptions, organizing, reminders, and making sure the medications are taken.  

  6. On Demand and After hours On Demand Visits: For existing clients who have unscheduled visit requests. Based on Provider availability.

  7. 1 or 2 hours of assistance with Companion care:  Added to any visit and as time permits includes sitting, engaging and interacting with the Client in the home or a facility, meal preparation or picking up meals, light housekeeping, laundry, organizing, prescription pick up, medication reminders, grocery pick up and putting groceries away (groceries must be ordered beforehand), setting up toiletries for AM/PM care, assisting with PT/OT/ST exercises, and fall prevention (The companion will walk side by side with the client, and always be near by the client to prevent falls.).

  8. 2 hours or 4 hours of assistance with Companion care only: As time permits, includes engaging and interacting with the Client in the home or a facility, meal preparation or picking up meals, light housekeeping, laundry, organizing, prescription pick up, medication reminders, grocery pick up and putting groceries away (groceries must be ordered beforehand), setting up toiletries for AM/PM care, assisting with PT/OT/ST exercises, and fall prevention (The companion will walk side by side with the client, and always be near by the client to prevent falls.).

  9. Dog/Cat Care: Add to any visit.  The Provider will make sure your pet has food, clean water, replace kitty litter for cats, and take your dog for a walk. 

  10. A 30 minute Acupressure chair massage added to any visit. Enjoy a massage that will focus on your pressure points to provide a release in tension, relaxation, and to assist in pain management.​​​
